Completion of this form is an integral part of participation in the St. Albert Athletic Programs.  The Athletic Association uses these forms to see how well we have met the goals we've set that the coaches have shared with you at the beginning of the season.  By receiving complete and constructive returned Evaluation Forms from each participating athlete's family, this allows us to continually improve and provide the best sporting experience for your child.
